Disease Management

CMS provides a comprehensive one-on-one approach to disease management with services provided by experienced clinicians.  Nurse Care Managers build strong relationships with your members to promote and teach self-management skills and coordinate their healthcare treatments.  Whereas many companies promote a “population management approach”, CMS encourages a one-on-one relationship with the member managing all of their disease processes and health factors that could be negatively affecting their well-being.  Care managers assess each participant individually and focus on behavior change over time as participants are taught to manage their disease state(s) more effectively.
