CMS case managers empower the healthcare consumer.  Our clinicians assist all parties in addressing options for care, goals and reimbursement issues prior to receiving care empowering participants and providers to make informed decisions that are cost effective and appropriate.  The participant and family are an integral component of the team.  Appropriate and frequent contact with the participant, family and health care providers ensures their participation and understanding of the plan of care.

Our clinicians will ensure that complex cases are managed efficiently. We will act as coordinators and liaisons between all interested parties, enhancing communication and promoting teamwork resulting in the most positive outcomes achievable.
