In the United States today, Diabetes currently affects about 1 in 10 Americans and results in costs that exceed $327 billion annually. Diabetes is also a leading contributor to other conditions such as heart disease, stroke, kidney failure, nerve damage, dementia and some forms of cancer. Effective support and diabetic member management offers opportunity to control concurrent and future health care costs, while facilitating positive health outcomes for the member. Through CMSI’s remote diabetic monitoring program, members receive a free glucose monitor, test strips and lancets. Blood testing data is seamlessly transmitted to a nursing surveillance team ready to support members with education and consultation when needed most. Members have the ability to share their testing data with their healthcare provider team as well as can take advantage of education, and text and chat alerts.
